usually there's no hmm slope-intercept form for it per se, the equation will be like the one in the picture below.

y = mx + b

Find the slope first:

y1 - y2 9 - 4 5

______ = _______ = ____ = is undefined

x1 - x2 3 - 3 0

so

y = 5/0x
