Calculate m using the slope formula

m =
(y_(2)-y_(1) )/(x_(2)-x_(1) )

with (x₁, y₁ ) = (2, 12 ) and (x₂, y₂ ) = (0, 4) ← 2 ordered pairs from the table

m =
(4-12)/(0-2) =
(-8)/(-2) = 4

y = 7x + 4 ← is in slope- intercept form

with slope m = 7

Difference between rates of change = 7 - 4 = 3
