  • y=√x

Translation

1 units left

  • y=√x+1

Compression of 1/4

  • y=1/4√x+1

Up 2 units.

  • y=1/4√x+1 +2

Answer
sqrt(x+1)/4+2
The /4 compresses the graph vertically by a factor of 1/4, the +2 moves the graph up by 2 units, and the +1 inside the radical moves the graph to the left by1 unit.
