The slope intercept form is


y=mx+b

Where

b is the y-coordinate of the y-intercept

m is the slope of the line

To determine the equation given that we know the slope and one point of the line you have to use the point slope form


y-y_1=m(x-x_1)

m=1/4, and point (12,-3)


\begin{gathered} y-(-3)=(1)/(4)(x-12) \\ y+3=(1)/(4)x-3 \\ y=(1)/(4)x-3-3 \\ y=(1)/(4)x-6 \end{gathered}

The equation of the line with slope 1/4 that crosses the point (12,-3) in slope intercept form is:


y=(1)/(4)x-6
