You just need two points to graph a line.

When x = 1

y - 4 = -1/4(1 - 5)
y - 4 = -1/4(-4)
y - 4 = 4/4
y - 4 = 1
y = 5

So, we have our first point, (1,5)

When x = 2

y - 4 = -1/4(2 - 5)
y - 4 = -1/4(-3)
y - 4 = 3/4
y = 4 3/4

Now we have the point (2, 4 3/4)

Now just run a line through the two points and you're done!
