How do you find slope and y-intercept form in a word problem?

you can find slope by using slope formula which is
m = (y2-y1)/(x2-x1)
To find y intercept form, use given point and slope and put it in this form,
y = mx+c

In the equation of a straight line, the slope is the number "m" that is multiplied on the x, and "b" is the y-intercept, where the line crosses the y-axis. This useful form of the line equation is sensibly named the "slope-intercept form."
