Point Slope form is
y - y1 = m(x-x1)
where,
m = slope
x1 = x coordinate of point the line goes through
y1 = y coordinate of point the line goes through

In this case,
m = 4
x1 = 4
y1 = -4

So,
y - y1 = m(x - x1)
y - y1 = 4(x - x1) ... replace m with 4
y - y1 = 4(x - 4) ... replace x1 with 4
y - (-4) = 4(x - 4) ... replace y1 with -4
y + 4 = 4(x - 4) ... rewrite y-(-4) as y+4
