and y-intercept is find by put x=0 in equation


y=-0.25(0-4)^(2)-2


y=-0.25(4)^(2)-2


y=-0.25(16)-2


y=-4-2


y= -6

Therefore, the vertex is (4, -2) and y-intercept is -6
