point slope form: y - y1 = m (x - x1)

replace y1 with the y point (6), m with the slope (-5), and x1 with the x point (9)

and you have your point slope form:

y - 6 = -5 (x - 9)

slope intercept: y = mx + b

just simplify the point slope equation by distributing -5 to the x and -9

y - 6 = -5x + 45

add the 6 to both sides

and you have your slope intercept form:

y = -5x + 51
