where
$m$ is the slope of the line and

$ (x_1,y_1) = (-4,8) $

Substituting in the equation:


$ y - 8 = (-1)/(4) (x + 4) $


$ \implies 4y - 32 = -x - 4 $


$ \implies x + 4y - 28 = 0 $
