use point-slope formula [y-y1 = m(x-x1) ] with the given info

P=(x1,y1) = (0,2) and slope m = -1/4

y-2=-1/4(x-0)

y-2= -1/4x

now try to move things till you get the slope-intercept form [y=mx+b]

y= -1/4x+2
