Slope Intercept Form is "y = mx + b"

What it means?

"m" - slope

"b" - y intercept (0,b)

So, we are given the slope, but not the y intercept..

For now, lets fill in the slope.

y = 1/4x + b

We are given a coordinate pair (-2,-1)

Fill it into the equation and solve for b.

-1 = 0.25(-2) + b

-1 = -0.50 + b

- 0.50 = b

We now have the value of "B"

Fill in the rest of the equation:

y = 1/4x + -.50

^ I think that is correct, sorry if it's wrong.
