You prepare a sterile IV medication in a clean room, ISO class 7 and inside that room is an ISO class 5 area - either an area that achieves this or inside a primary engineering control. A PEC is a laminar air flow hood - either horizontal or vertical. Or, if you do not have an ISO class 7 area, you can use a biological safety cabinet or Compounding Aseptic (CAI) and Containment Isolators (CACI) that can be certified to use a room that is less than ISO class 7. The only reason you would ever prepare a sterile IV medication on a counter is in an emergency situation for "immediate use." Immediate use is defined as the entire contents will be used within 60 minutes of the preparation.
