y = -.25x + 12

Explanation:

Subtract x on both sides.

y = -.25x + 12

There is your answer. The "1" before the y doesn't need to be written, as a rule in algebra, ones do need to be written and are assumed to be there if there is no number at all.

-.25 is the slope and 12 is the y-intercept.
