a) y=mx+b is slope intercept form.

b) the m is the slope of the line.

c) the b is the y-intercept

d) the x-axis is horizontal, the y-axis is vertical.

e) m = rise/run = y2-y1/x2-x1

f) pick 2 points on the graph & determine their coordinates. find the difference in the y-coordinates (the rise) & the difference in the x-coordinates (the run) & divide them.
