You forgot to add the photo of the graph but here are some equations I hope help:
Slope: y2 - y1
-———
X2 - X1
Y = MX + B
Y - Y1 = m ( X - X1)
M=slope
B=y-intercept
X=input
Y=output
F(x)= y
