HELP PLEASE!

I need help with writing a subject for an email. This is the information about the email that needs to be written.
You are emailing customer service on a website about your super-expensive new headphones that you bought after saving up your snow shoveling money. You weren’t being rough on them, but it’s only been a month and they won’t connect to your phone through Bluetooth anymore. You ordered them online and have to contact them through the website to figure out this mess. (Question: Do you want your money back or an exchange? Hint: You might start with “Dear ________ customer service person” or something like that if you do not know the person’s name).

here is what i would use as a format for the customer service box.

I would like either an exchange or money back, as the pair of headphones was expenisve for me. I found the headphones to be quiet fraglie and no longer connect to my (Apple or Andriod phone) via. BlueTooth,

I ordered them online on (instert webside name/address), I hope we could reslove this issue in regards, (insert name)
