Answer:

Chapter 11:

There doesn't seem to really be any...

Chapter 12:

Katniss learns that District 4 has rebelled against the capital

  • District 4's major industry is fishing
  • Panem now has no seafood

Katniss suspects that District 8 is rebelling.

Haymitch tells Katniss about rumors of rebellion is District 7 and 11

Chapter 14:

No rebellion here. This chapter takes place on the train to the capital for the Quarter Quell.

Chapter 15:

No rebellion. Mostly just introducing the other tributes.

Chapter 16:

In their individual demonstrations to the Gamekeepers, Peeta does something to upset them (in the next chapter, we find out he pained a picture of Rue right after her death).

When Katniss goes in, she takes a target dummy and hangs it from a noose. She uses berry juice to write "Seneca Crane" on it.

These are both acts of rebellion.

Chapter 17:

Flickerman is interviewing the tributes right before they are sent off to the game. Many of the tributes express their anger about the Quell and make subtle (but effective) comments about Snow.

Katniss was forced to wear a wedding dress chosen by Snow. Cinna tells her he made some alterations. He tells her to twirl when the time is right. When she twirls during the interview, her dress catches fire and turns into a black feather dress that makes her look like a mockingjay. This was an act of rebellion by Cinna against the government.
