y = a {(x - h)}^(2) + k


vertex = (h \: \: \: k)

from the table


vertex = ( - 2 \: \: \: 4)

therefore


h = - 2 \: \: and \: \: k = 4


y = a {(x + 2)}^(2) + 4

when x= 0, y = 3


3 = a {(2)}^(2) + 4


3 = 4a + 4


a = ( - 1)/(4)

therefore equation of the function


y = - (1)/(4) {(x + 2)}^(2) + 4
