Right now the line's equation is written in standard form, ax+by=c.

First, subtract 12x on both sides:-

4y=-36-12x

Now divide by 4 on both sides:-

On further simplification,

switch -9 and -3x places (it's not mandatory)

y=-3x-9

That's our equation in slope intercept form.
