To find the slope of the perpendicular line we just take the reciprocal and therefore change the sign of slope,

Perpendicular to y = -½x + 6

Hence, the slope will be = -½ = 2

Now we have to find the slope-intercept form, So

here y intercept is given and the point will be (0,b) then eqs. of line,

=> y = mx + b [slope-intercept form]

=> -4 = 2*5 + b

=> -4 = 10 + b

=> b = -14
