y - 4 = 4 (x + 3)

Explanation:

Point-slope form is y - y1 = m (x - x1)

y1 is 4, m is 4, and x1 is -3

When you plug them in, you get: y - 4 = 4 (x + 3)

Note: (x - -3) becomes (x + 3)
