Answer:y-h = m(x-k) where m=slope and (k,h) is a point on the line, just plug in (2,5) as k=5 and h=2 and m=4

y-5 =4(x-2)

that's point slope form of the equation

for slope intercept form just expand and simplify combining the constant terms on the right

y = 4x -8 + 5

y = 4x -3, where -3 is the y intercept
