Throughout the entire play, Demetrius is in love with Helena, who's thrilled at the turn of events and doesn't really question why his regard for Hermia has changed. Demetrius says only that his love for Hermia has faded and he now loves Helena. The pair are wedded later in the play. However, in Act 2, scene 1, Demetrius tells Helena to stop following him since he does not love her, and promises to kiII Lysander.
