Lets first put all the known information into the point-slope form:

y-y_(0) =m(x-x_(0) )

  • (x_(0),y_(0) ) = (-4,-5)
  • m (slope) : 1/2

(y+5)=(1)/(2)(x+4) \\y + 4 = (1)/(2) x+2\\

To put it into slope-intercept form, we must solve for y:

y = (1)/(2) x -2

Hope that helps!
