0=4x+y-5

Explanation:

slope(m)=-4

y-intercept(c)=5

now, the equation joining the straight line satisfy the equation,

y=mx+c

or, y= -4x+5

or, 4x+y-5=0

or, 0=4x+y-5

it is the required equation.
