The slope-intercept form of an equation of a line: y=mx+b

m - slope

b - y-intercept

We have the points (5, -4) and (-1, 8). Substitute:

m=
(8-(-4))/(-1-5) =
(12)/(-6) =-2

Put the value of the slope and the coordinates of the point (5, -4) to the equation of a line,

-4=-2(5)+b

=-10+b
