the point-slope form is

y - y1 = a(x - x1)

with "a" being the slope, (x1, y1) being a point on the line.

so, we have here

y - 1/2 = 4(x - 3/8)

simplifying this gives us then the slope-intercept form

y = ax + b

with b being the y-intercept.

so, we get

y - 1/2 = 4x - 4×3/8 = 4x - 3/2

y = 4x - 3/2 + 1/2 = 4x - 2/2 = 4x - 1

so,

the first, second and forth statements are true, the third is false.
