Slope-intercept form:
y=mx+b where m is the slope and b is the y-intercept

1) Determine the slope (m)

To find the slope, take two points from the table,
(x_1,y_1) and
(x_2,y_2) and plug them into the following equation to solve for m:

m=(y_2-y_1)/(x_2-x_1)

Plug m into
y=mx+b .

2) Determine the y-intercept (b)

Take any point given in the table and plug them into
y=mx+b (along with the slope that you solved for earlier) as
(x,y). Solve for b. Plug b back into
y=mx+b along with the slope.

I hope this helps!
