hello i think the question is asking you about how mentally ill characters are often used in horror to be evil or scary and what you think about that

so for example you could say that in the horror film split they use a character with DID (dissociative identity disorder) as a villain and then that reflects badly on real people with that mental disorder

theres also always the idea of a "scary asylum" in horror films (like gothika is set in one) and people who've "escaped the asylum" are always killers (like in the film halloween but i forgot the antagonists name sorry haha) but in real life people have to stay at hospitals for their mental health all the time and more often than not theyre not murders

in terms of do you think you societys understanding of mental health has improved you could say YES because its more accessible to get therapy and less stigmatised/people wont judge you as much, celebrities talk openly about mental health when in the past they wouldnt, people are generally more understanding

(i suppose you could say no too but i cant think of as many reasons for that)
