How did you identify the values of the slope and the y-intercept of each equation?

Assuming the equations are in the format y=mx+b, then mx is always your slope, and b is always the y-intercept. If your equation is in standard form, like this, ax+by=c, all you have to do is convert it to slope-intercept form
