This piece of art is trying to show us that medics are superheroes. The way that the medic has his head tilting down, and has a hand to his face, speaks to how they(and others) question how important their job is, especially because they are so often criticized for doing their job incorrectly. However, the shadow illustrates that this is not the case. The shadow is demonstrating reality. The shadow has a cape on, which illustrates that medics have an important job and are "superheroes." This is supported by the fact that the shadow's posture is significantly better, demonstrating that in actuality, medics have an important job, which they do very well, and they should be proud of their work.
