Answer is y= 1/2x + 3

Step by step

We know slope intercept form is
y =mx + b

m = slope
b = y intercept

We find slope on the graph by creating a slope triangle as shown on the attachment.

We find rise 3 and run 6, slope is rise over run so 3/6 simplified to 1/2

We find the y intercept is at 3 on the y axis

So y = 1/2x + 3 is your answer
