The point-slope form is:

y - y1 = m(x - x1)

where m is the slope and (x1, y1) is a point on the line.

Then, the table is:

Slope point point-slope form

-3 (12,5) y - 5 = -3(x - 12)

6 (4,-5) y - (-5) = 6(x - 4)

y + 5 = 6(x - 4)

3 (-6,-2) y - (-2) = 3(x - (-6))

y + 2 = 3(x + 6)

1/2 (7,-6) y - (-6) = 1/2(x - 7)

y + 6 = 1/2(x - 7)

-3/4 (-8,2) y - 2 = -3/4(x - (-8))

y - 2 = -3/4(x + 8)
