To find the vertex form, we have to find a


\begin{gathered} y=a(x-h)^2+k \\ 14=a(0-(-3))^2+5 \\ 14-5=9a \\ a=(9)/(9)=1 \end{gathered}

Hence, the vertex form is


y=(x+3)^2+5

The standard form is obtained by developing the square power


\begin{gathered} y=x^2+6x+9+5 \\ y=x^2+6x+14 \end{gathered}

Hence, the standard form is
