From the vertex equation, now we are looking for an equation of the form -

y = a(x-4)^2 + 5

And passes through 1,2 , so

2 = a(1-4)^2 + 5

2 = 9a +5 or -3 = 9a

Or a = -1/3

So the equation is,

y = -1/3(x-4)^2 + 5
