The line is straight so format would be:
y = mx + c
The line goes through 2 on the y axis so c = 2
Gradient is change in y / change in x so
(5 - 2) / (4 - 0) = 3/4 = 0.75
m = 0.75
Equation is y = 0.75x + 2
