(y_(2) - y_(1))/(x_(2) - x_(1)) = slope . y2 would stand for the y coordinate of the second point, y1 would stand for the y coordinate of the first point. While x2 would stand for the x coordinate of the second point and x1 for the x coordinate of the first point. Once plugged in, it should look like this:
