So, basically I can't remember how to do this the long way but I used a calculator on Desmos (the graphing one to be specific). You should be able to find tutorials on how to use it and graph problems to find their answers but if not, you can just message me and I'll walk you through how to use it.

As for the form it asked for in the question, slope-intercept form is just:

y = mx + b

m is the slope, and b is the y-intercept (the place where the line crosses the y-axis)
