The moment that the reader realizes that Death was not an omniscient character is when the author reveals that the person who the helper was scared of was not death, but just someone with a surprised face. Death probably appeared to the servant as a woman in the Baghdad because he doesn't want the person to know he's Death right away, so he could take him secretly? Hope this helps lol
