2. Is it important for medical practices to check a patient's balance before the patient comes in for a scheduled appointment

No! It is not a medical practice's responsibility to ensure a patient has the means to pay for care prior to a scheduled visit. Medical practices are to provide care to whichever patients enter their doors, regardless of perceived medical status, insurance status, financial status, or any other ways that can be considered discriminatory -- this would include checking a patient's balance.
