An orange is a really unusual image to use when insulting a person. What do you think Shakespeare is trying to say with this image? Is he teaching something bigger?

I think Shakespeare used the image of an orange to show that just like an orange, a person's true nature is hidden beneath their outer layer. He was teaching the idea that people are often not what they seem on the surface, and that there is always more to a person than meets the eye.
