f(x)=(x+4)^2 -3

Explanation:

First using -b/2a find the x value of the vertex

In this case that is equal to -4

Then plug -4 in as the x value to find the corresponding y value which in this scenario is equal to -3

Now all we need to do is find the a value which is the number that modifies the x^2 part of the equation (in standard form), which in this case is 1.

Putting it all together you get your final solution ( all of this is shown step by step in attatched picture)
