c) ready to change: in this stage the person has made a decision to change and is actively preparing to take action. strategies that could be used to support change at this stage include setting specific goals and developing an action plan and providing social support and encouragement and offering rewards or incentives for progress.

d) action to change: in this stage the person is actively changing their behavior. strategies that could be used to support change at this stage include providing ongoing feedback and support and helping the person to overcome obstacles or setbacks and reinforcing positive changes through rewards or recognition.
