Given:

The table of values.

To find:

The equation in slope intercept form.

Solution:

From the given table it is clear that the line passes through the points (5,17), (8,41) and (12,73).

Consider any two points from the table, i.e., (5,17) and (8,41). So, the equation of line is

y-y_1=(y_2-y_1)/(x_2-x_1)(x-x_1)

y-17=(41-17)/(8-5)(x-5)

y-17=(24)/(3)(x-5)

y-17=8(x-5)

y=8x-40+17

y=8x-23

Therefore, the equation of line in slope intercept form is
y=8x-23.
