I think the answer would be Y=3x+4

Using this formula, m= (y1-y2)/(x1-x2)

= (31/4 - 25/4)/(5/4 - 3/4) = 3/2÷1/2 = 3

y=mx+b

25/4=9/4+b

b=4

And now you fill out the format, y=mx+b with your newfound values of m and b,

y=3x+4

From this function, you can input any real number (x) and get your corresponding value (y).
