The constitution is unclear about two things related to replacing a president. What are they?

In the Constitution it is not clear is the Vice President would become President if the President were to die, resign, be removed from office, or become disabled.

The second is what should happen if a vise-president vacancy arises. The Constitution did not provide a filling for such vacancy.
