A local company, OrthoSC, is needing a new contact portal for people who are interested in their services but are not current patients. Our point of contact at the company has several issues with their current system and would like a new system to solve these issues and to better streamline the process for end users. You will need to create a form that will allow the user to enter their contact information, as well as what their issues are and if they wish to join the company newsletter. This page should utilize the format of their current webpage, though you are welcome to adjust items if you think it would increase the aesthetics of the site. All user data entered into the site should be kept so it can be viewed by the staff. In addition to this contact page, there should also be an administrative page within a separate folder off the main website that will allow the administrative staff to view all the information that has been entered into the system. They should also be allowed to search the entered information as well as edit the information in the system. If needed, they will also need to add a new potential patient into the system. For this site there are several milestones that must be completed.

They are as follows:
1: For this you will review the supplied request above and create a list of all the information you think the user should enter on the form.
