Answer : y=6x+6
the formula is y=mx+b
m in mx is always slope so we know what slope is so we just plug it in
and same thing for b, b is always y intercept so you just plug it in and get y=6x+6

slope intercept form: y = mx + b where m = slope and b = y-intercept.

you know that:

m = 6

y-intercept = 6

slope intercept form: y = 6x + 6
