To convert the standard form to slope intercept form, you need to clear y from the standard form


\begin{gathered} 4x+3y=12 \\ 3y=-4x+12 \\ y=(-4x+12)/(3) \\ y=-(4)/(3)x+4 \\ \end{gathered}

That is the slope intercept form of 4x+3y=12
